Through MyMedicare, the Australian Government is providing additional funding to general practices to support more personalised, ongoing care for patients.

At Polaris Medical Centre, we encourage our patients to register so we can continue delivering high-quality, coordinated care tailored to your needs.

What is MyMedicare?

MyMedicare is a voluntary patient registration program that strengthens the relationship between you, your GP, and your regular medical centre.

By registering with Polaris Medical Centre, you are formally nominating us as your primary care team. This allows us to better understand your health needs and provide more consistent, proactive care.

Benefits of Registering

When you register with Polaris Medical Centre, you may benefit from:

• Longer Medicare-funded telephone (telehealth) consultations
• Increased access to bulk billed longer telehealth consultations for:
  • Children under 16
  • Commonwealth concession card holders
• Improved continuity of care with your regular GP and clinical team
• Better coordination of care for chronic or complex conditions
• Access to new Medicare-funded services as they become available

Additional benefits include:

• Aged care residents: More regular GP visits and enhanced care planning
• Chronic disease patients: Improved connection between hospital and GP care

Who is Eligible?

You can register for MyMedicare if you:

• Have a valid Medicare card or Department of Veterans’ Affairs (DVA) card
• Have attended Polaris Medical Centre:
  • At least 2 face-to-face visits in the past 24 months, or
  • 1 visit if living in a remote area

Exemptions apply for patients experiencing hardship, including homelessness or family violence.

Children & Young People:

• Parents/guardians must register children under 14
• Patients aged 14–17 can register themselves

How to Register

You can register in one of the following ways:

1. Online:
   • Via your Medicare Online Account
   • Or the Express Plus Medicare app
2. In person:
   • Complete a registration form at reception
